The big mouth at the back is his mouth. His mouth is located in his eyes, in other words his eyes were replaced by his mouth and that he has only a mouth in his face. As you can see in the drawing, he’s blind folded, that is where his mouth is.

I’ll make some revisions about this character sometime soon.
